Clearfield Community Nurses

Clearfield Community Nurses (CCN), a division of Helpmates, Inc., is a skilled home health agency that provides medical care and therapy in the comfort of your home. Whether recovering from surgery, illness, or an injury our services will put you back in control of your health. Through the use of our Telehealth Monitoring System, along with visits from nurses and therapists (depending on your needs), CCN can be the eyes and ears for your doctor. Regular contact with your doctor informs him/her of your progress.

Home Health Care services are provided by nurses, therapists, aides and other professionals in the comfort of your own home. Services provided include (but are not limited to): Nursing visits ; Rehabilitation Services (physical, occupational and speech therapy); Medical Social Services (visits from a licensed social worker if necessary for assessment, counseling and community resource planning); Certified Home Health Aides (assist with personal care, dressing, light housekeeping, etc.); Telehealth Monitoring (allows the nurse to view patient’s vital signs from the office such as pulse, blood pressure, etc.); Activity and Sensor Monitoring (tracks the individual’s activities of daily living to assist them in living safely and securely at home).

CCN is licensed by the PA Dept. of Health and is Medicare and Medicaid certified.
